Community Champion
POSITION TITLE: Community Champion

DEPARTMENT: Administration

REPORTS TO: CEO

STATUS: Part Time

Plays an influential role in creating a shift in their community toward healthier living. Key in helping their organization prepare an infrastructure for population health.

POSITION QUALIFICATIONS

Education required: Bachelor's degree or three years of experience in healthcare, communication education, or related field.
Demonstrates above-average verbal and written communication.
Must be able to effectively use computer systems.
Demonstrates the ability to function responsibly in a minimally supervised work situation and to utilize proven decision-making and problem-solving skills.
Demonstrates the ability to be a self-starter, have superior organizational skills, and attention to detail.
Demonstrates excellent team-building, interpersonal, and collaborative skills.
Demonstrates above-average leadership skills.
POSITION SKILLS & ABILITIES

Knowledge of common word programs.
Ability to problem solve and make recommendations for conflict resolution.
Ability to respond calmly and effectively in emergent situations.
Ability to prepare for and participate regularly in scheduled Center calls and communications.
Respond timely to Center communications.
Specific Duties

Supports the Mission, Vision, and Values of RMC.
Acts as a liaison for DRCHSD staff for community care coordination planning.
Acts as a point of contact for DRCHSD staff and RMC.
Facilitates communications between parties.
Assists with ongoing community health needs assessments and monitoring of changes in community health outcomes as identified.
Reviews, understands, and presents the community health status report provided by DRCHSD staff to the Health Care Organization leadership teams and community partners.
Assists the executive team and community partners through community care coordination planning activities.
Consults with the executive team to schedule community care coordination events and workshops.
Builds awareness of Health Care Organization services within the community.
Engages community partners in community events.
Coordinates and hosts community care coordination events and workshops.
Assists in the development and implementation of a community care coordination plan.
Supports the implementation of the community care coordination action plans.
Supports community care coordination events by facilitating workshops, conversations, and presentations.
Community Champion Learning

Annual summits, workshops, bootcamps

DRCHSD Program Webinars (supportive of community care coordination planning)
National and regional conferences
DRCHSD Staff Communication

Prepare for and participate in regularly scheduled Center calls.
Respond timely to Center communications.
Progress Reporting

Submit quarterly progress report programmatic checklist to DRCHSD staff and RMC Leadership.
Participate in quarterly calls to share action plan(s) implementation and progress of community care coordination plans.
Engage Community and Build Relationships

Conduct community outreach to local health and social service providers.
Build community support and gain buy-in for program activities.
Act as a catalyst for culture change.
Promote health and wellness.
Assist in improving community health outcomes.
Share parish health ranking with the community.
Promote RMC services and quality of care.
Build awareness of locally available services.
Assist in the development of short videos of the community.
Provide monthly updates to community partners and other community members, organizations, and agencies defined by leadership.
Program Consultations and Assessments

Support the RMC team through program activities.
Observe action planning sessions to support community care coordination planning.
Participate as requested by the executive team in post-project follow-up activities.
Support the executive team with the implementation of recommendations in relation to community care coordination planning and as applicable to their role and skill.
Participate in program evaluation activities as applicable to their role.
Perform other duties as assigned or requested.
